Immunization Requirements for 7th and 11th grade

Attention Parents/Guardians of Next Year’s 7th&12th Grade Students

As a reminder, there are immunization requirements for 7th and 11th grade

Students Attending 7th grade MUST HAVE the following immunizations for the 2020– 2021 school year:

*1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
*1 dose of meningococcal conjugate vaccine (MCV)
Also, all students must have 4 doses of Polio on file (they can have 3, if the last one was given after their 4th birthday)

Students Attending 12th grade MUST HAVE the following immunizations for the 2020– 2021 school year:

*A Second dose of meningococcal conjugate vaccine (MCV), for a total of two
Also, all students must have 4 doses of Polio on file (they can have 3, if the last one was given after their 4th birthday)

Please note: Your child may have received these immunizations at a recent appointment, but we do not have a copy of them on file at school. New guidelines for immunizations went into affect August 1, 2017. Students must have proof of these immunizations to enter 7th or 11th grade.
